SELF-CARE DEFICIT NURSING THEORY 2 Self-Care Deficit Nursing Theory Operationally and Theoretically Description of the Concept The concepts are operationally and theoretically defined in self-care deficit nursing theory or also known as Orem’s theory of nursing. The major concepts of nursing theories that are essential for their metaparadigm are in the environment, health, nursing, and a person. Orem’s theory has indicated nursing concept, the science and art of nursing which is interventions of nursing. The theory states where the professional knowledge of nurses is applied when taking care of a patient, by technical or procedural skills and also when to use physical touch. Concerning the concept of a person, the theory has indicated the activities or practice that an individual performs are initiates in maintaining their health. The ability of a human to engage in self-care is conditioned by development state, age, life exercise, fitness, available resources and sociocultural orientation (Smith et al. 2015). The Orem’s theory of nursing also utilizes the concept of health which is relative to the person and is defined according to the perspective of the patient. Every individual has a different view of determining health. Healthiness refers to the level of wellness that a patient considers such as psychological, physical, intellectual, spiritual, and emotional. Theory of Nursing Systems describes how the self-care of the patient is the responsibility of a nurse. The model indicates that when a person capability for self-care is not sufficient for therapeutic self-care demand, the nurse compensates the deficit for demand care. Orem’s theory of nursing also describes the concept of an environment which consists of social, external, and internal factors that impact the health of patients. According to Orem, the environmental components are enthronement elements and factors, developed environment and conditions. The theory has been used in the education of nurses, and the developed comments can be applied to teaching, research, and practice. Orem’s philosophy has served as a framework of conceptual in associate degree programs in nursing schools. Three levels of preventions have applied Self-Care deficit nursing theory; primary, secondary, and tertiary. Primary has utilized the approach when nursing care is aimed at universal self-care or developmental and when they are therapeutic. Tertiary or secondary has used the theory when the attention of nurse is aimed at self-care healthdeviation. Orem’s approach has a marginal utilization in practices of the clinic and less extent in education, research, and administration (Mohammadpour et al. 2015) Orem identified five methods of helping, guiding others, doing and acting for others, teaching another, and provide a personal development promoting the environment to achieve future demands. In the educative system, Orem’s theory is used when patients can take care of themselves, but they need assistance when making decisions, knowledge acquisition skills, control of the behavior. In practice, the theory can also be used when a patient can cater of part of their self-need but require the assistance of a nurse to help with the other tasks; both the patient and nurse assist each in performing self-care. An example is a patient who has undergone abdominal surgery. The theory supports the educative system of a nurse required wholly because of the inability of an individual who is unable to care for him or herself. When in such a situation, the care of a nurse is needed to assist in accomplishing all self-care rudiments. Nurses have to assess the ability of the individual to provide self-care and context of the environment of the person to overcome the limitations associated with health (Berbiglia et al. 2013). Combination of the three sub-theories namely; theory of self-care, self-care deficit, and nursing systems and the critical concepts generate a general self-care model, which outline the nursing process of Orem’s. The method determines the deficiencies of self-care and defines the nurse or person’s roles in acquiring demands of self-care. The model has assisted in developing the three steps used by nurses which are in comparison to the nursing process that is currently taught — the first in assessment where nursing is needed to collect data on people and physician perspective of the patient’s health. Nursing diagnosis where nurses are supposed to design a system is partly compensatory to education that is supportive in implementation and evaluation stage where nurses assist the family and the patient in selfcare requirement to attain goal, to attain good health and related outcome. The theory ensures that the results achieved can be compared to the finding that was planned. Outcomes or Consequences Orem’s general nursing theory has a significant effect on clinical practice. One of the limitations of the theory is that it is oriented to illness and it cannot be used in a wellness setting.
